RN Triage Positions – Bay Area Locations & Remote!

Position Summary

The Remote Triage Nurse is a licensed Registered Nurse (RN) who provides remote clinical assessment, patient education, triage, and care coordination services via phone, video, or secure messaging platforms. This role supports patients by evaluating symptoms, prioritizing care needs, and guiding them to the appropriate level of care from a virtual setting. Working remotely, the Triage Nurse supports Bay Area Community Health’s mission by ensuring patients receive timely and appropriate care, reducing unnecessary emergency department use, and promoting continuity of care.

Key Responsibilities

  • Conduct telephone or video-based assessments to evaluate patient symptoms and determine urgency of care for adult and pediatric patients using standardized clinical protocols.
  • Utilizing critical thinking to assign triage levels, provide evidence-based recommendations, and refer to appropriate care settings (e.g., clinic appointment, urgent care, ED)
  • Document triage assessments, nursing interventions, and patient instructions accurately in the electronic health record (EHR)
  • Collaborate with physicians, advanced practice providers, and care teams to coordinate timely follow-up
  • Escalate high-risk situations to clinical leadership as appropriate
  • Educate patients on chronic disease management, medications, and health promotion
  • Participate in quality improvement initiatives and regular staff meetings via video conference
  • Maintain current knowledge of clinical protocols and community health resources

Minimum Qualifications

  • Current and unrestricted RN license in the State of California
  • Minimum of 2 years of clinical nursing experience, with preference for outpatient, family medicine, urgent care, emergency or medical/surgical nursing
  • Strong critical thinking, clinical judgment, decision-making skills, communication, and documentation skills
  • Proficient in using EHR systems and telehealth technologies, preferably EPIC.
  • Ability to work independently in a fast-paced, remote setting.
  • Must have a secure and private home workspace with reliable internet

Preferred Skills:

  • Certification in Telephone Triage or Ambulatory Care Nursing (e.g., AAACN).
  • Bilingual capabilities English and Spanish, Vietnamese, or Mandarin highly desirable
  • Experience in telephone triage or remote nursing strongly preferred
  • Experience with chronic disease management or behavioral health.

Work Environment & Schedule

  • Fully remote position; may require periodic in-person training or meetings • Flexible scheduling may include evenings or weekends • Requires ability to sit, talk, and listen for extended periods and use computer equipment
