We have an opportunity for 1 x part-time (22.5 hours per week), Care Coordinator based at Royal Primary Care East sites, based in Chesterfield. An opportunity has arisen to join our growing team at Royal Primary Care and we are looking for an enthusiastic part-time Care Coordinator to work within in our busy medical practice within the Chesterfield area focusing on supporting patients who are in respite care at a local Care Home. The successful candidate's main responsibilities will be to manage the registrations and prescription management of a busy respite care home. This role will require excellent IT skills, good prioritisation skills and an understanding of General Practice processes or relevant transferable skills. To work within the Administration team with a focus on achieving excellent patient care via the telephone and face to face. The successful candidate's main responsibilities will be to manage the Registrations and Prescription management of a busy respite care home. Providing professional and holistic administration support for all patients who are discharged to the home ensuring a smooth and safe transition from hospital to respite to home. Assist in delivering high-quality administrative support to enable effective patient care in the care home environment. Requirements / Skills
Excellent IT skills, good prioritisation skills and an understanding of General Practice processes or relevant transferable skills. Ability to work within an Administration team and contribute to excellent patient care. Other essential skills include the ability to work as part of a team, and a good level of qualification (or equivalent experience) and competency in Maths and English. Additional information
